NOTICE: MAILING CHECKS
As of June 20, 2016, we will no longer accept HSA contribution or transfer checks at our Salt Lake City P.O. boxes. After this date, contribution and transfer checks sent to this address may experience significant delays in processing or be returned to the sender, unprocessed. If you need to send a contribution or transfer check, please sign in to our secure website and download the appropriate forms to see the correct address to send it to. (Address varies based on your account type.)
